The Background Security Clearance form is the first step in the clearance process, and collects basic personal information about the applicant and their immediate family. Please note that the witness signature on the front page is required for all applications - background cannot be consented to without a witness signature.
The Integrity and Lifestyle Questionnaire is a personal evaluation that all applicants must complete in an honest and truthful capacity. The purpose of the document is to assess an applicant's lifestyle, integrity, and overall aptitude to be considered as a potential DPD staff member. Police officers and police staff go through identical assessments at this stage in the process.
As part of the clearance process, police staff applicants will be required to provide a list of 10 references; a combination of employment references (direct supervisors, colleagues etc.) and personal reference (no family). The following information is required for each reference, and can be filled in the document:

• Relationship to you (as much details as possible)
• How long have you known the individual
• Telephone Number
• Email address
