  There are three versions of the form. Select the form appropriate for your purposes:
  • General (e.g. non-volunteer, employment, practicum, adoption)
  • Volunteer
  • Police Certificate (for use outside Canada - e.g. visas, immigration, etc., when requested by foreign governments)

  In addition to completing one of the forms above, if you are applying for a position with a person or organization responsible for the well-being of one or more children, seniors, or physically or mentally challenged persons, complete:
